Abramson Center Receives 5 Star Rating from the Centers of Medicare and Medicaid

The Abramson Residence is proud to announce that it has received an overall rating of five stars by the Centers for Medicare and Medicaid Services (CMS). The results are posted at medicare.gov/nhcompare.

This achievement is a result of the sustained efforts by many Abramson Center staff members to improve quality measures. Over the past few years, CMS has expanded the list to include a focus on short-term rehabilitation outcomes such as functional improvement and avoidance of re-hospitalization.

To meet these challenges, the Abramson Center developed teams which focused on these and other quality areas. There are four ways that staff has had a direct impact:

  1. Preventing negative outcomes. Falls and skin breakdowns are just two health concerns for residents of skilled nursing facilities. In order to prevent accidents and injuries, our staff is dedicated to making regular rounds, tending to residents’ personal needs in a timely manner, and keeping residents engaged in activities.
  2. Maintaining functional abilities. Whether through an exercise group or a restorative nursing program, maintaining the highest possible ability to walk, transfer, or eat independently fosters a healthy sense of dignity and well-being for older adults.
  3. Detecting early decline. Quickly reporting any changes in a resident or patient’s condition is the responsibility of all Abramson Center staff. This allows us to put interventions in place to help prevent further decline or an unnecessary hospitalization.   
  4. Promoting patient self-efficacy. The Abramson Center places high importance on making sure that our short-term rehabilitation patients with a chronic condition, such as hypertension, diabetes and heart failure, are able to manage their disease after discharge. Our staff educates patients on healthy lifestyle decisions as well as identification of early-warning signs to reduce the odds that a patient will end up back at the hospital.
