style="color:#666699;">What is transitional care?</span><br /> Transitional care is short-term nursing, medical, and rehabilitative care designed to help patients get stronger and be more fully recovered after hospitalization, but before they return home. Transitional care bridges the gap between the hospital setting and home, and encourages a patient s successful movement toward greater independence. </p> <p><span style="color:#666699;">Where is transitional care provided?</span><br /> Transitional care is most often provided by nursing care centers, though some hospitals also offer transitional care. </p> <p><span style="color:#666699;">How can I benefit from transitional care?</span><br /> Transitional care can help you manage pain, build strength, gain confidence, and recover more fully before you return home. </p> <p><span style="color:#666699;">Where is your program located?</span><br /> Birnhak Transitional Care is located on the campus of the Abramson Center for Jewish Life in a designated center consisting of 27 private rooms, state-of-the-art rehabilitation gym, therapy kitchen, living room and dining area, all with beautiful views of our outdoor gardens. The Abramson Center is located in Horsham Township, Montgomery County, about 30 minutes from Center City, the Main Line, Lower Bucks County and the northern suburbs of Montgomery County, and provides free parking. <br /><br /> Our amenities include: <ul> <li>All private rooms</li> <li>Flat-screen TV</li> <li>Telephone</li> <li>Wireless Internet access</li> <li>Emergency call system</li> <li>Bathing suite</li> <li>Laundry service</li> <li>Beauty/barber services</li> </ul> </p> <p><span style="color:#666699;">What services do you provide?</span><br /> Patients receive the services of our comprehensive transitional care team including: a medical director who directs all rehabilitative care and is board certified in Internal Medicine, a physiatrist who is board certified in Physical Medicine and Rehabilitation; registered nurses and nurse practitioners; certified nursing assistants; physical, occupational, and speech therapists; social workers; dietitians; and chaplains. <br /><br /> Frequent assessments and reviews of each patient s clinical care and treatment plan are done throughout the patient s stay.
